We are seeking Registered Nurse to join our team in our Kensington (Philadelphia, PA) location.

 

This role is full-time, Monday - Friday 12p - 8p.

 

Earn $44/hr.!

 

 Position Details

The RN is responsible for nursing care for individuals receiving treatment/assessment in the PAD location at 265 E Lehigh Ave.

Additional responsibilities will include:

Provide ongoing patient monitoring, whole person care, administers medications as prescribed and appropriately communicates ongoing patient needs to the medical team as well as all provider/city agencies involved in an individuals care Participate in multidisciplinary collaboration and appropriate clinical utilization of a system of care Collaborate with all PAD Healthy Village staff to ensure ongoing whole person care occurs; improving an individuals social determinants of health (SDOH) Conduct admission assessments through interviews with patients and relatives to gather case history Plan and provides nursing care for patients to include wound care and any medical recommendations regarding an individuals care. Make nursing diagnosis of the medical status of patients and recommends treatment options. Consult with medical and other behavioral healthcare staff in designing appropriate treatment plans for clients Complete timely and accurate patient monitoring of vitals and baseline screeners (COWS, CIWA, Falls Assessment, Pain Scale, etc) Arrange further medical attention when necessary Assist in maintaining a safe and secure environment for staff and patients Assist in patient education regarding substance use diagnosis and treatment Facilitate patient education sessions on medical topics inclusive but not limited to: HIV, Infectious Disease, MAT options, tobacco cessation, harm reduction Document all patient information on medical charts and follows physician’s written orders Maintain patient confidentiality
