Short Description
Additional responsibilities to the Current Population Care Coordinator Position
Provide care coordination and management for all patients enrolled in Cooper’s Remote patient monitoring program within their scope and standards of practice
Triages patients with change in clinical status and follows escalation of care pathway
Utilizes disease specific clinical protocols to enhance patient care and outcomes
The purpose of the role is to support the implementation and sustainability of the new remote patient monitoring program a Cooper. It is intended to enhance patient clinical care outcomes, patient satisfaction, and provision of quality care. It contributes to Cooper as it affords an opportunity to compete with the market as a care management service, afford a potential downstream revenue-keep patients within the system, and increase patient provider relationship.
