Short Description

The Cooper Health System has recently established Cooper University Dental Health, a new comprehensive dental care program located on campus of Cooper University Hospital at the Center for Comprehensive Health, 3 Cooper Plaza in Camden, NJ. The Center for Comprehensive Health is a multidisciplinary health center that has a strong and well-established reputation for state-of-the-art patient care that now includes comprehensive oral health – general dentistry, dental hygiene, and outpatient oral surgery. We are seeking a Population Health Coach to offer comprehensive guidance to patients in all facets of dental care. This role entails assisting patients in understanding treatment plans and coordinating dental appointments, ultimately supporting them throughout their treatment process within our program

Provides programmatic and client support for the provision of patient-centered care . 

Encourage continuity of care between clinician and patient, by providing individual support and assistance to the patient and family in navigating the health care system.

Demonstrate superior organizational & interpersonal skills; will be detail oriented; will have proven ability to work collaboratively, and be able to multi-task.

Current Registered Drivers License in State of NJ necessary.  


Experience Required

3+ years of work experience in a dental organization or hospital system preferred.
Knowledge of health promotion, nutrition, and counseling preferred (specific disease and lifestyle related topics ideal).
Strong interpersonal skills; demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.
Ability to take direction, follow through with tasks, and work independently. 
Flexibility, optimism, and a cooperative working style.
Fluency in Spanish preferred. 


Education Requirements

Associates degree (preferably in health science) OR equivalent healthcare experience such as EIP CSR – NMCM
Bachelor’s degree preferred


License/Certification Requirements

Registered and valid drivers license


Special Requirements

Required NJDHSTS HIV and NMCM Clinical trainings/courses.

Provides programmatic support and organization of a Patient Centered Medical Home.
Establish contact with patients with chronic care needs to ensure their understanding, agreement, and participation in wellness.
Demonstrate superior organizational & interpersonal skills; will be detail oriented; will have proven ability to work collaboratively and be able to multi-task.
Facilitate the patient’s process of gaining the knowledge, skills, tools, and confidence to become active participants in their care.
Encourage continuity of care between clinician and patient, by providing individual support and assistance to the patient and family in navigating the health care system.
Continually update tracking mechanisms for program reporting purposes.
