Job Summary
The Population Health Consultant serves as a trusted advisor, cultivating in-depth relationships with assigned practices and regional partners through in-person meetings and strategic engagement. This role focuses on partnering with Clinical Integrated Network (CIN) and MSSP ACO practices to support providers and clinical teams in achieving quality goals and cost savings goals.
The consultant is responsible for implementing strategies that promote the principles of value-based care, maximizing quality outcomes and return on investment (ROI) across multiple payor programs.
Responsibilities
· Act as the primary representative and point of contact between the Vandalia Health Network and participating member sites. This includes fostering regular collaboration with Practice Directors, Medical Directors, Quality Champions, and Practice Coordinators to align efforts with Network goals and ensure engagement with key initiatives
· Facilitate monthly practice quality meetings, primarily conducted in person, and provide hands-on working sessions as needed for additional support.
· Provide training and education to assigned practices to increases provider education and support resources to help improve quality metrics.
· Perform ongoing in-depth analyses of practice performance reports and other data to identify opportunities, develop practice-specific action plans, and implement strategies.
· Serve as the liaison between the Network and participating member sites for other programs by facilitating introduction and coordination of these services within the practice.
· Facilitates the implementation of population health strategies including community and patient outreach for gap closure.
· Serve as the subject matter expert (SME) to practices with the use of population health management tools. Provide training and education as needed to ensure utilization and understanding of the analytics technology.
· Participate in the development of presentations and education materials needed to optimize VBR and MSSP ACO performance.
· Deliver relevant data and recommendations to Vandalia Health Network executive leadership.
Required
· Bachelor’s or Associate’s degree in a relevant field (E.g., Business or Health Science)
· Successful account management experience is preferred, with a focus on strategic relationship building, cross-functional collaboration, and delivering measurable results.
· At least one year of experience working independently with minimal supervision and meet project deadlines.
· Demonstrated ability to manage multiple projects simultaneously, prioritize tasks effectively, and consistently meet deadlines.
· Prefer experience with population health management principles or related clinical quality or practice operations.
· EMR proficiency and data analytics experience preferred.
