Population Health Coordinator - Part Time
Overview With support and guidance from supervisors, coordinates internal and/or external health partnerships and project teams to plan and implement population health initiatives. Responsibilities * Develops and implements project assessment activities with guidance from supervisors and other partners. * Supports and provides technical assistance to one or more collaborative partnerships. * Collaboratively works with internal and external stakeholders to plan evidence-based/best practice projects. * Supports and coordinates efforts of internal and external stakeholders to implement evidence based practices, activities and programs. * Develops and implements project evaluation activities with guidance from supervisors and other partners. * Coaches and mentors project partners/peers and Project Assistants * Represents Dartmouth-Hitchcock in all interactions with internal and external stakeholders * Performs other duties as required or assigned. Qualifications * Bachelors Degree or equivalent experience. * Minimum of 1-2 years’ experience in related field * Strong interpersonal and organizational skills, creativity, flexibility, diplomacy, and the ability to work as part of and contribute to a positive team culture. * Demonstrated ability to handle multiple tasks and coordinate projects under pressure with support.
