In consultation with the Population Health Manager, the Population Health Team Lead maintains direct oversight for the development, implementation, and monitoring of Population Health team and activities across our Oregon regions. This includes all lines of business: Commercial, ACA, ACO, Medicare Advantage and Medicaid CCOs across the state of Oregon. This position provides leadership and supervision to the Population Health team through offerings of subject matter expertise, ongoing coaching/guidance, and liaising of regional initiatives complimentary of Pacificsource strategies. This includes transformation strategy and creative brainstorming within our communities as well as community focused work plans, including integrated care, support of contractual initiatives, with a focus on the delivery of high quality of care within our communities.

Essential Responsibilities:

Accountable to provide supervision, training, and support for Population Health team.Conduct performance management and assist Population Health Manager with hiring, staff development, and ongoing support of team members.Guides the Population Health Team project management and strategic implementation of processes and changes for improvement to PacificSource across all lines of business.Consults with our regional Strategists in the design, organization, implementation, and evaluation of strategies and initiatives that align with the Pacificsouce Strategic Plan to improve health in all our communities.Elevate regional provider innovations to the Population Health Manager to inform contract development and negotiations with contracted Providers.Partner with HEDIS, STARS, Risk Adjustment department, and leadership internally on key initiatives to partner with provider partnerKeeping updated on all CMS, OHA, HHS and all governmental requirements of Risk Adjustment and Quality measures for all lines of business.Ability to consult with all levels of business.Create ongoing opportunities for team engagement, development, support and employee advancement

Supporting Responsibilities:

Meet department and company performance and attendance expectations.Follow the PacificSource privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.Perform other duties as assigned.

SUCCESS PROFILE

Work Experience: A minimum of five years’ experience in healthcare with emphasis on Population Health as a whole, patient- and family-centered care, healthcare delivery operations, provider practice operations, project management and/or supervisory (i.e. supporting people or implementation of strategy and/or programs with coaching/teaching required. Minimum of 3 years or program(s) development preferred.

Education, Certificates, Licenses: Bachelor’s degree in healthcare administration, population health, or similar field is required, or equivalent work experience. Master’s degree or clinical licensure preferred.

Knowledge: Knowledge/experience with quality improvement initiatives in the clinic setting including clinical quality outcomes and patient experience. Knowledge of national and state quality measures such as CMS Stars, NCQA, HEDIS, CAHPS/HOS required. Strong computer skills using Word, Excel, and PowerPoint. Experience with medical billing, claims processing systems, data analytics, and health care information strongly preferred. Knowledge and experience implementing continuous improvement efforts or LEAN concepts strongly preferred. Must have strong analytical, influencing, and problem-solving skills. Understanding of quality initiatives, evidence-based medicine and care coordination required. Demonstrated ability to effectively communicate with all levels of a staff and management including executive leaders. Proven ability to learn new healthcare information systems and work with multiple business systems. Must be self-motivated, organized, and detailed oriented.

Environment: Work inside in a general office setting with ergonomically configured equipment. Travel is required approximately 25% of the time.
