Position Description
The Population Health Care Supervisor, under the direction of the Manager of Population Health, has primary responsibility for care management activities for a specific patient panel. This will include identifying the high acuity patient population and developing, conducting, and monitoring care management processes. In addition, maintains responsibility for supervising assigned staff.
Position Requirements
REQUIRED
Bachelor of Science Degree in Nursing and a minimum of 3 years of experience in a clinical setting OR Associates Degree in Nursing with a Bachelor of Science Degree in a health related field with a minimum of 5 years’ experience in a clinical setting Proficient computer skills, including Microsoft Office (specifically Word and Excel) Excellent oral and written communication skills. Good organizational, analytical, and problem-solving skills. Ability to function effectively with a wide variety of disciplines, including physicians and nurses. A team player that can follow a system and protocol to achieve a common goal. Confidence to communicate and collaborate with other community health care organizations and personnel. Able to participate as part of a team to accomplish an assigned project. Demonstrated ability to set priorities and make appropriate clinical and supervisory decisions. Must possess good problem solving skills.
PREFERRED
Previous experience in a Primary Care Setting. Previous experience in case management, care coordination, or patient navigation. Previous population health experience.
