Post Acute Resource Specialist - Full Time Day Shift - Traverse City, MI
SUMMARY The Post-Acute Coordinator (PAC) is accountable for facilitating, implementing, and confirming post-acute transition plans for the patient population. The position will respond to requests for post-acute services from inpatient clinicians and community-based providers. ENTRY REQUIREMENTS High school diploma or equivalent is required. Associate’s/Bachelor’s degree in Social Work or related field is preferred. 2 yrs of an equivalent combination of education and experience in a hospital/medical related field is required. Experience with the patient discharge process and post-hospitalization placement is preferred Social work technician license preferred. Effective interpersonal communication skills are expected. Must be able to demonstrate the ability to prioritize tasks. Knowledge of medical terminology preferred. ORGANIZATION Reports to the Manager of Patient Care Management. SPECIFIC DUTIES 1. Meets expectations outlined in Commitment To My Co-workers. 5. Uses effective customer service/interpersonal skills at all times. Acts as a receptionist for individuals having contact with the CM team, including telephone and personal contacts. 6. Partners with the Care Managers or other clinical associates to prioritize and implement post-acute placement in a SNF, rehab, sub-acute, assisted living facility or other venue as ordered. 7. Facilitates prompt processing of all requests for post-acute services including durable medical equipment (DME), and appointments with community resources. 8. Confirms patients’ benefits, identify preferred providers and use knowledge of community resource availability to effect timely disposition. 9. Identifies availability, completes and transmits necessary forms to implement and complete the discharge and placement process. Uses electronic software to distribute post-acute placement requests to community and regional resources and. 10. Establishes and maintains routine communication with referring Care Managers, Hospitalists or other prescribing member to keep them informed of the progress of their request. 11. Identifies obstacles to timely transition and collaborate with the Case Manager to resolve. 12. In collaboration with requesting clinician, arrange safe transportation that best fits the patient’s needs and financial resources. 13. Updates all involved parties regarding progress, revisions and other information related to implementing the plan. 14. Maintains log of any quality issues encountered with preferred providers that require follow-up. 15. Resolves any pre-certification, registration or form completion prior to setting up an appointment with a community provider. 16. May participate in follow-up processes to discharged patients to query about quality and timeliness of post-acute arrangements. 17. Demonstrates independent problem solving skills on behalf of the Care Management team and the patient. 18. Coordinates and schedules post-discharge medical appointments and enters information into the EMR. 19. Uses fax, email, photocopy and phones to securely transmit information to agencies. 20. Professionally respond to payer’s request for information. 21. Demonstrates effective time management skills. 22. Demonstrates knowledge of and adherence to departmental policies. 23. Collaborates with area Skilled nursing facilities, maintains a daily listing of post-acute bed availability. 24. Maintains and organizes departmental statistical data. 25. Promotes program stability, reputation, and growth by coordinating work with colleagues. 26. Appropriately receives constructive guidance from leadership staff. 27. Preserve the basic human rights of expression, decision, action and personal dignity for all patients and families. 28. Respects the cultural diversity of all patients, families and employees of the hospital. 29. Demonstrates mature negotiation and conflict management strategies and skills. 30. Demonstrates effective use of critical thinking skills to independently solve problems. 31. Demonstrates knowledge of current regulatory and accrediting agency requirements as it pertains to obtaining services for patients. 32. Attends and participates in team building activities, daily huddles, patient care conferences, and departmental meetings. 33. If social work technician license or Bachelor’s in Social work license (must be supervised by a Master’s prepared Social Worker or RN/MSW Patient Care Manager), may also perform these duties: a. Interviewing clients to obtain information about the client’s situation and to provide info about service and community resources b. Conducting case-finding c. Monitoring client’s compliance with a program d. Provide life skills training 34. Performs other duties as required
