Position Summary / Career Interest:The Organ Transplant Coordinator Lead for the heart transplant and VAD program coordinates activities and oversees the daily operations of the clinical care team. Serves as a professional role model, leader and daily problem solver for the team. Collaborates with others to establish evidence based standards of care for the patient populations served. Primary responsibilities are directly involved with coordinating the interdisciplinary team approach in the evaluation, treatment, and facilitation of the heart failure/transplant process, and follow-up care in individuals with a heart transplant and/or a ventricular assist device.

Responsibilities and Essential Job FunctionsWorks with the heart transplant and VAD team in setting and achieving goals for the clinical operations of the heart transplant and VAD programs. Directs and coordinates the clinical management of patient care under the supervision of the cardio-thoracic surgeon and transplant cardiologist. Provides training and support for new heart transplant and VAD coordinators and other support staff. Assures open and clear communication with patients, their families and the health care team members. Educates patient / family regarding VAD and transplant. Develops, plans, conducts, and evaluates educational activities for patients, families, professionals and the public. Engages in activities designed to encourage the participation of medical facilities, referral sources and health care team members in the transplant and VAD process. Develops and oversees QAPI program. Leads all QAPI planning, Morbidity and Mortality, PDSA, best practice and protocol review and development. Responds to call 24 hours a day. Assists with the surgical implantation and explanation of VAD’s. Rounds daily on inpatients with VADs. Coordinates and manages comprehensive clinical care of the patient throughout hospitalization. Supervises professional and nursing support staff involved in patient care. Develops, collects, records, and maintains UNOS and VAD data registries. Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department. Required Education and ExperienceBachelor Degree Nursing from an accredited School of Nursing 3 or more years of heart transplant/VAD experience
Preferred Education and ExperienceMaster Degree Nursing
Required Licensure and CertificationLicensed Registered Nurse (LRN) - Single State - State Board of Nursing Must have current active multistate state Register Nurse (RN) license in good standing in state in which you live OR active single state RN license in good standing in the state you will be working with proof of application of a multistate RN license in the state in which you live. Staff hired prior to 2/1/2024 are required to obtain a multistate state RN license from the state in which they live at the time of their next RN license renewal by 3/31/2026. CPR/ AED/ BLS - Other BLS CPR
