Job SummaryCoordinates screening, admissions, outreach, and/or PPS Coordination for Inpatient Rehabilitation Facility (IRF) services. Works with Rehab Leadership, physicians, care management staff, and multidisciplinary team to expedite the patient's care/transfer, analyzing cost, quality and utilization to facilitate the admission and PPS transmittals, shares ownership with the attending physician and Rehab leadership to expedite patients' correct level of care, services and financial considerations.
Essential FunctionsCoordinates input from all health professionals, conducts assessment of patient and family needs and formulates a documented admission/transfer plan assuring continuity of care.Communicates daily with physicians, rehab management, hospital staff, families, third party payers, transmittal vendors and patients to facilitate coordination of clinical activities and to complete and enhance a seamless transition from one inpatient setting to alternative levels of care and associated transmittal.Serves as subject matter expert for IRF admission rules and regulations, SNF admission rules and regulations, and PPS transmittals.Conducts outreach activities including coordination and rounding with key acute care units and programs.Oversees quality and financial reporting to meet the needs of BHT and PMR administration and rehab QA teams.Ensures unit maintains compliance with IRF 60/40 rule in accordance with BHT and PMR leadership.Documents all pre-screening interventions in the medical record according to regulatory, legal and case coordination requirements. Documents the transfer plan utilization management software and other programs as appropriate.Completes all regulatory required transmittal documentation within required timeframes.QualificationsRequired
At least one License and/or Certification in area of specialty - Healthcare license Upon HirePreferred
