This Opportunity:Monarch is searching for a Primary Care Advanced Practice Provider (FNP, AGNP or PA) for a Part Time position, 20 hours per week offering a flexible schedule anytime between Mon-Fri. No weekend hours available. This is a community based role traveling to the homes of the people we serve. Option to work 5-10 hours per day with the potential for a Full Time role in the future with Outpatient Primary Care after these services are implemented within the organization.The Primary Care Provider (Advanced Practice Provider – Physician Assistant or Nurse Practitioner) delivers high-quality medical care in the community, primarily through home visits for individuals receiving Assertive Community Treatment Team (ACTT) services.
This role complements behavioral health services by addressing the physical health needs of individuals with Serious and Persistent Mental Illness (SPMI). Responsibilities include conducting assessments, managing chronic conditions, prescribing and coordinating care, and promoting preventive health practices. The APP collaborates closely with ACTT staff, behavioral health clinicians, and community partners to ensure integrated, person-centered care. The position emphasizes outreach, relationship-building, and continuity of care to reduce barriers, improve wellness, and minimize avoidable hospital and emergency department use.What You'll Do:
Deliver Primary Care in the Community: Provide assessments, preventive screenings, chronic disease management, acute care, and urgent interventions in homes and community settings.
Clinical Management: Conduct physical exams, review prior treatments, order diagnostic tests/referrals, prescribe medications, and develop person-centered care plans in accordance with best practices and protocols.
Education & Engagement: Provide health education to individuals and families, supporting understanding of diagnoses, treatment options, and preventive practices.
Collaboration & Coordination: Partner with ACTT staff, behavioral health providers, hospitals, primary care clinics, public health departments, and community agencies to ensure integrated, whole-person care.
Continuity of Care: Support transitions from hospital or emergency settings back into the community and participate in care planning for high-risk or complex individuals.
Documentation & Compliance: Maintain timely, accurate records in the Electronic Health Record (EHR); adhere to all agency policies, procedures, and training requirements.
Professional Development: Attend required meetings and trainings; maintain certifications and licensure.
Other Duties: Perform additional responsibilities as assigned; travel required for community-based care delivery.

Community-based position with frequent travel for home and field visits
Part-time, 20 hours per week with potential for full-time hours if combined with primary care outpatient clinic services
Flexible scheduling may be considered to meet patient and agency needs
