POSITION STANDARDS Unrestricted NH Medical License Required Current DEA license Required BLS Certification Require Board Certified or Board Eligible in Family Medicine. If Board eligible, must be actively working toward Board Certification. Demonstrated Competence in providing comprehensive family medicine as delineated on clinical privilege list. Position Objective Provides primary care services to Adult and Pediatric individuals and their families. This provider is not expected to carry a primary panel of continuity patients or participate in on-call duties (?)

 

Performance Expectation Provide Face to Face services (or Telehealth as appropriate) for Same Day Patients in Primary Care Evaluate all assigned adult patients presenting to the Primary Care Clinic. Obtain an appropriate history and physical exam. Order and interpret diagnostic laboratory and imaging studies. Provide appropriate treatment to all assigned patients presenting to Primary Care. Obtain consultation as clinically indicated and appropriate. Document the history, physical exam, and laboratory and imaging studies as well as treatment in the medical record. Obtain appropriate informed consent for all procedures as described in APD Medical Staff Bylaws, Rules and Regulations and Policies. Complete all records within 7 days of day of service. Provide consulting providers with complete and accurate medical records. Communicate in a timely and collegial manner. Review previously ordered laboratory and imaging studies and determine appropriate course of treatment for patient based on results. Communicate results to patients in a timely manner. Document all “after-hours” communication with patients. Treat all patients and staff with respect. Participate in staff education and clinical Quality Assurance/Performance Improvement activities. Attend clinic staff meetings. Abide by CMS Conditions of Participation for Hospitals. Abide by the APD Medical Staff bylaws, Rules and Regulations and policies. Abide by all APD policies Provide in-basket coverage for providers out of the clinic requiring coverage for their absence. Receive assignment of list of providers needing coverage during scheduled shift. Team with nursing and other clinical staff to prioritize and review, act upon and close messages received via the Results, RX Requests, Patient Calls, Staff Messages and myDH Advice Requests categories in the assigned providers’ inbox. Be available to nursing staff to address urgent triage needs, especially as they apply to requests for same day appointments. Perform Paxlovid consent calls, or other pandemic-related treatment consents.
