Physician Affiliate Group of New York (PAGNY) is adding a Primary Care Physician to the group at NYC Health + Hospitals/South Brooklyn Health. Accommodating more than 250,000 outpatient visits, and more than 15,000 inpatient admissions annually. The hospital maintains a strong academic affiliation with SUNY Downstate Health Sciences University and NYIT College Of Osteopathic Medicine.

Attend to a direct patient panel of adult primary care patients Ensure patient/provider continuity and access to timely and appropriate levels of care. Educate, mentor and supervise residents and medical students Work collaboratively with mid-level providers Participate in system-wide performance improvement and quality management projects as needed
