Physician Affiliate Group of New York (PAGNY) is adding an Primary Care Physician to the group at NYC Health + Hospitals/Harlem. The Adult Clinic at NYC Health + Hospitals/Harlem is designed to promote wellness and prevent disease and disability. Our physicians are nationally recognized experts in their fields. Special programs/support groups for those with chronic illness are held to provide additional help for diabetes, cancer, amputee education & Alzheimer’s/Dementia. The hospital maintains an academic affiliation with Columbia University Vagelos College of Physicians and Surgeons.

The service at Harlem Hospital is an academic practice sponsored by the Columbia University Vagelos College of Physicians and Surgeons Outpatient care, clinical supervision of fellows, residents and medical students Practice has daily patient load & time with patient/quality patient care is promoted  Weekly team meeting to encourage collaboration and new ideas  The practice is a research site for diabetes & fully staffed with support staff such as social workers, clinical pharmacists & dietician
