Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Position Overview :
The Geriatric Primary Care Program Manager will provide essential project management and administrative support to our Geriatrics Primary care medical group. This role is pivotal in ensuring smooth daily operations, facilitating communication, and scheduling for the team. This role will report directly to the SVP of Clinical Practice.
Schedule: Full time position, regular business hours. O ccasional travel to Wisconsin and Florida (and other markets as added). Hybrid role: position will be based out of our offices in downtown Stillwater, MN (2-3 days per week in office & 2-3 days per week working remote from home)
Salary: $70,000 - $80,000. Salary will be commensurate with experience.
Responsibilities :
+ Project / Program Management
+ Lead or support special projects related to patient care initiatives, process improvements, or operational efficiencies
+ Track project timelines and milestones; Document all decisions and changes made
+ Lead the implementation and the communication of approved practice changes
+ Measure the impact of process improvements and policy changes
+ Administrative Support:
+ Prepare, organize, and facilitate team meetings in 90io
+ Support with creation of slide decks, training materials, and playbooks
+ Coordinate meeting sites
+ Coordinate travel arrangements for team members as needed
+ Documentation and Organization
+ Keep the team Google drive organized and maintain relevant documents
+ Prepare agendas and keep meeting minutes
+ Support with scheduling and on-call management
+ Data and Reporting
+ Gather and summarize data as requested
Qualifications :
Education/Certification/Experience
+ Bachelor’s degree or higher
+ Two or more years of experience in an administrative support or project management role
+ Experience in a healthcare organization preferred
Knowledge/Skills/Abilities
+ Knowledge and experience with Assisted Living and Memory Care communities a plus
+ Ability to work independently with excellent time-management and organizational skills
+ Ability to maintain professional relationships members of the care team
+ Ability to communicate effectively and professionally, both verbally and in writing, with diverse populations
+ Advanced-level of computer proficiency with email, fax, word processing, spreadsheets, and databases
+ Excellent customer service skills
+ Demonstrated compatibility with Bluestone’s mission and operating philosophies
+ Demonstrated ability to read, write, speak, and understand the English language
