Key Responsibilities:

Coordinate and implement Individual Support Plans (ISPs) for assigned clients in their homesTrain and oversee staff on clients' medical needs, medications, personal preferences, and compliance requirementsEnsure quality care by maintaining detailed documentation, including daily notes, goals, and health trackingSchedule and accompany clients to medical appointmentsMaintain safe, clean, and healthy living environments, including proper food storage and safety equipmentManage client funds, ensuring accurate record-keeping and proper documentationCollaborate with Program Coordinators on quality management initiatives and follow-up careCoordinate staff schedules and provide shift coverage as neededConduct and document required safety drillsMonitor and manage home mileage budgetsParticipate in support plan meetings and service monitoring visits

Education and/or Experience Qualifications:

High school diploma or GED requiredOne year experience in intellectual/developmental disabilities field OR successful completion of Missouri Quality Outcomes training (approved by Division of DD regional office)

Additional Qualifications:

Must be 18 years of ageCurrent driver's license with acceptable driving record and current auto insurance requiredStrong communication and interpersonal skillsAbility to read and interpret documents such as safety rules and procedure manualsBasic math skills and ability to maintain accurate recordsMust have reliable means of communicationBackground check clearance must be obtained, including criminal record, driving record, abuse/neglect, and fingerprint checkMust complete New Hire Orientation at the beginning of employmentMust complete all training requirements including Relias at the beginning of employment and annually thereafter
