Job Summary:

As a Primary Support Professional, you will work closely with clients in their homes and communities, coordinating daily activities and ensuring their needs are met. Your role will involve training staff, overseeing health and safety standards, and participating in the development of positive behavioral support strategies.

Key Responsibilities:
Work directly with assigned clients in their homes or communities, as designated by the Program Coordinator and/or Regional Director.
Participate in Individual Support Plan meetings and RN visits or service monitoring visits when possible.
Train staff on each client’s medical needs, personal preferences, and compliance.
Ensure staff are knowledgeable of client medications and potential side effects through effective communication.
Oversee daily residential notes, goals, and health tracking.
Review the physical condition of the home to ensure health, safety, and cleanliness standards are met.
Ensure safety kits are present and complete, including First Aid Kits and Emergency Preparedness Kits.
Review MARS for signatures and errors per shift.
Complete event reports in accordance with state guidelines.
Schedule staff or cover shifts for personnel who are absent.
Track mileage used in homes to stay within budget.
Participate in the development of positive behavioral support as needed.
Ensure clients are taken to all required medical appointments.
Maintain receipts for purchases made from client funds and record them appropriately.
Ensure money counts are completed at each shift change by staff.
Collaborate with the Program Coordinator on follow-ups from monthly RN visits and trainings.
Ensure safety drills are conducted according to the Individual Safety Plan.
Document daily exceptions and submit them to the Program Coordinator at the end of each month.
Perform other duties as assigned.
Education, Experience, and/or Credential Qualifications:
Must be 18 years of age; must have a high school diploma or GED.
One year of experience in the field of intellectual/developmental disabilities, or successful completion of training in the Missouri Quality Outcomes approved by the Division of DD regional office.
Additional Qualifications:
Must have a reliable means of communication.
Must possess skills to read and understand written materials.
Must possess skills to write clearly and legibly.
Physical Requirements:
Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects.
Sedentary work involves sitting most of the time but may involve walking or standing for brief periods.
