Adoration Health is seeking a skilled Licensed Practical Nurse (LPN) for a full-time position in Hornbeak, TN. This role involves providing nighttime care for a patient requiring trach and G-tube management. The schedule fluctuates between Sunday to Friday or Monday to Friday, depending on the family's needs. Schedule:

50 hours per weekFluctuating between Sunday-Friday or Monday-FridayNight shifts: 8:00 PM - 6:00 AM

Responsibilities:

Provide high-quality nursing care, including trach and G-tube management, in a home care setting.Monitor and document patient conditions, ensuring timely communication with the healthcare team.Administer medications and perform routine nursing procedures.Collaborate with the patient’s family and healthcare providers to ensure comprehensive care.

Responsibilities Perform selected nursing procedures within the scope of LPN practiceProvide patient and family teaching according to plan outlined by the RNMonitor patient's vital signs and make specific measurements and observationsMonitor and record intake and output.
