PRN Private Duty Nurse (LPN/RN) - Shift Work

As a Private Duty Nurse, you will play a pivotal role in improving the lives of our patients and their families. You’ll provide high-quality, comprehensive nursing care while working closely with the interdisciplinary team to ensure patient-centered care.

Responsibilities

Deliver skilled nursing care in the client’s home under the direction of a Registered Nurse (for LPNs) or autonomously (for RNs).

Participate in the development and execution of patient care plans in collaboration with the interdisciplinary team.

Provide health education to patients and caregivers to promote understanding and participation in care.

Accept and document verbal and written orders from physicians, ensuring RN Case Manager co-signature when required.

Communicate effectively with physicians, patients, families, and team members regarding patient concerns and changes in condition.

Accurately document all care provided in a timely manner.

Follow up with physicians, patients, and caregivers as needed to address concerns and ensure continuity of care.

Requirements

Active nursing license in the state of Connecticut or a compact license (LPN/RN).

Minimum 6 months of RN Pediatric experience preferred (RN only).

Home health experience preferred for both LPNs and RNs.

Reliable transportation to perform job duties.

Ability to lift, push, pull up to 50-100 lbs.
