Website: coastalharbor.com The Case Manager coordinates non-therapy needs for residents, including case management, staffing, discharge planning, and utilization review, by coordinating with Therapists and other departments regarding residents’ needs. This includes communicating on a regular basis with external contacts, including referral sources, state-appointed guardians, and other agencies related to residents’ care.
