What You’ll Do:

✔ Deliver skilled, compassionate nursing care in the patient’s home under the direction of a Registered Nurse.

✔ Develop and execute individualized patient care plans in collaboration with an interdisciplinary team.

✔ Educate and empower patients and caregivers to actively participate in their care journey.

✔ Accept and document physician orders accurately, ensuring co-signature from an RN Case Manager as required.

✔ Communicate effectively with physicians, patients, families, and the care team regarding patient needs and condition changes.

✔ Maintain precise and timely documentation of all care provided.

✔ Ensure continuity of care by following up with physicians, patients, and caregivers as needed.

What You’ll Need:

✔ Active nursing license in the state of Connecticut or a compact license (LPN).

✔ Minimum of 1 year of nursing experience, Home health experience

✔ Reliable transportation to perform job duties effectively.

✔ Ability to lift, push, or pull 50-100 lbs to assist with patient care needs.

✔ A passion for delivering high-quality, compassionate care with professionalism and empathy.
