This is a PRN/As Needed role. Shift hours are 6:30a - 6:00p Monday - Friday. No weekends or holidays! Must be willing to float to all three QuadMed Wisconsin locations: Sussex, Lomira and West Allis.GENERAL PURPOSE OF JOB
The Pharmacist is responsible for the timely and effective provision of pharmaceutical care by means of review of patient therapy; interpretation and processing of medication orders; monitoring and managing drug therapy; discharge counseling to assure the continuum of care; monitoring and supporting the drug distribution system; and the provision of frontline drug information support for other healthcare team members.
KEY RESPONSIBILITIES
Dispense pharmaceuticals and limited Durable Medical Equipment as prescribedPrepare or supervise the preparation of liquid medicines, ointments, powders, tablets, and other medicationsCheck prescriptions for correct dosage and for drug interactions and incompatibilitiesVerify patient information for every new and refilled prescriptionCompound prescribed medications not commercially available when necessaryEnsure proper storage of pharmaceuticals stocked in the pharmacyWork collaboratively with providers to provide optimal pharmaceutical careProvide patient education/consultation to customers for all new and refilled prescriptions and on OTC (over the counter) medicines as requiredPerform order entry of prescriptions into the pharmacy database when technicians are not presentContact physicians and benefits providers as necessary to process a prescriptionProvide a professional and confidential environment for customers on all occasionsMaintain strict confidentiality of PHI and adhere to HIPAA guidelinesJOB REQUIREMENTS
Education:
Bachelor of Science in PharmacyDoctor of Pharmacy Degree preferredExperience:
Minimum of five (5) years of experience in a clinical setting if no DoctorateCertificates, Licenses, Registrations:
Active pharmacist license in the state of practiceCurrent CPR (BLS) certification or must be obtained within 60 days of hireKnowledge, Skills & Abilities:
