Role and Responsibilities Administer direct skilled nursing care to patients, in accordance with physicians orders. Essential Functions + Provide hands-on, skilled nursing care in accordance with physicians orders. + Provide health education to the patient and family. + Assess patient response to the plan of care interventions and communicate changes to the case manager. + Collaborate and share findings with the multidisciplinary care team. + Document those services rendered to the patient, note changes in the patients condition and/or family and home situations. + Document communication with case manager and physician. + Meet visit productivity requirements and conduct other home visits (SOC/ROC, recertifications, discharges) as requested by clinical manager. + Other duties as assigned.
