Position Summary:


We have an exciting opportunity to join our team as a Program Associate.

 

The program associate position in the Division of Rheumatology is based at Bellevue Hospital Center and will support the coordination of care for our patients. This position is a two-year, grant funded position to help promote patient-centered trauma informed (PCTI) care and will involve both learning, teaching and supporting the implementation of the PCTI patient approach. The PCTI approach focuses on person-centered supportive service delivery, which considers everyones experiences, needs, strengths, preferences, and goals. In addition, it emphasizes trauma-informed approaches to care in which all individuals and levels of an organization or system have a basic realization about trauma and understand its impacts on individuals, families, groups, organizations, and communities. Most Rheumatology patients are chronically ill and medically complex. Thoughtful support of their care coordination will improve the patient experience and relieve some of the burden patients experience from their illness. This is an opportunity to work closely with patients, improve health care delivery and implement patient-centered system improvements.

Job Responsibilities:

Support patient-centered trauma informed care training and implementation of PCTI practices by all patient care engaged physicians and staffSupport assessment of patients care experience in general and per PCTI methodContact patients pre and post visit to ensure that care instructions are understood and are feasible for the patient Ensure that follow up appointments are made across the specialty continuum and that patients were able to attend their visits and their care needs met Support medication fulfillment with prior authorization and infusion scheduling Support inpatient to outpatient care transitions Teach patients how to navigate the medical system and self-advocate, up to and including helping patient optimize their use of MyChart Support physician / Rheumatology directed messages to MyChart. Collaborate with our current administrative teams with respect to helping streamline patient visit schedules and show rate complianceCommunicate with physicians any concerns that are identified with respect to patient concerns and unmet care needs and work to help find solutions to identified gaps Participate in ongoing NYU Division of Rheumatology research projects and participate in development of novel projects potentially related to this position Participate in NYU Division of Rheumatology academic activities, such as weekly conference to support learning about subspecialty care and the practice of medicine Performs other duties as needed 

Minimum Qualifications:


To qualify you must have a Associates' Degree is required. Minimum of 1 Year of related experience in an office setting or the equivalent combination of education and experience. Effective Oral, written communication and interpersonal skills. Computer Skills: Working knowledge in the use of Microsoft Office suite of applications. Ability to develop basic formulae for spreadsheets; set up routine computer procedures; Ability to access e-mail, electronic calendars, and other basic office support software. Ability to proofread documents and write clear documentation. Ability to type and knowledge of standard office practices. Ability to multitask while paying attention to details. Ability to use standard office equipment such as copy machines, multi-line telephones, fax machines.

Preferred Qualifications:


Associate degree in a related field with 2 to 3 Years of related experience in an office setting is preferred. Prior case management/care coordination experience preferred. Interest in patient-centered trauma informed care and supporting instruction in PCTI is desirable.

Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $41,977.19 - $54,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
