Program Coordinator, Full Time - AMG Pulmonology Allergy & Sleep - Specialty - Summit

Under the direction of the Administrator of Practice Operations and the Project Manager, and in collaboration with program team members, the HGNS Program Coordinator and will be responsible for the coordination of the Atlantic Health Hypoglossal Nerve Stimulator (HGNS) Program’s patient pathway from referral to post implant care. The coordinator will work to enhance our ability to deliver high quality, safe, affordable patient care within a healing culture to improve the health status of the communities we serve based on PRIDE (Professionalism, Respect, Involvement, Dignity and Excellence).
Responsibilities:
Prompt and effective communication with patients, physicians, and project program team members, and others both inside and outside of AHS. Point of contact for HGN patients from pre-consultation through entire care pathway journey, initiating and returning communication in a timely manner to patients and physicians. Accurate, complete, and timely entry and maintenance of data in the program’s Master database, Mini dashboards and appropriate components of SleepSync database. Partners with Data Coordinator to review and update the HGN program dashboard to ensure data availability for metric reporting. Receives notifications from and screens prospective patients from the Inspire Advisor Care Program (ACP), HGNS and AMG websites, providers and patient self-referrals. Schedules patients for all time and service appropriate appointments with HGNS Program providers according to program criteria and standards. Manages patient scheduling and re-scheduling for initial sleep medicine consults or other required pre implant assessment, post implant, follow up and long-term care appointments for those patients receiving device implantation. Requests or provides appropriate documentation to ensure optimal continuity of care (i.e., medical records, sleep study results, compliance reports) and ensures scanned within the Electronic Medical Record. Assistance with completion of forms associated with the care pathway. Documentation of all interactions with patients and/or providers pertaining to care pathway in patient’s electronic medical record, HGNS Program dashboard and/or other application Appropriately escalates questions or issues to program administration. Effectively communicates with patients, families, peers, members of the multidisciplinary healthcare team, community organizations and resources both verbally and in writing. Facilitate communication among physicians, staff and administration establishing a climate that encourages teamwork and promotes collegial collaboration among all team.
