Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.
But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
Reporting to the Assistant Director, Tenant 360, the Program Coordinator, Tenant 360 provides time-limited case management services focused on housing retention, resource development, and community integration intended to support older adults with aging safely in the community . The Program Coordinator provides information and referral services; benefits application and navigation assistance ; resource development support; and community integration programming.
The case management is typically 3–6 months of engagement and on a referral basis.
Depending on program need, this position may carry a small caseload of more complex clients.
This position has a 35-hour work week. This is a fully in-person position that will divide time across the Breaking Ground building portfolio. The position may require occasional evening or weekend hours to support tenant events.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
+ Provide consultation throughout Breaking Ground programs as it specifically relates to seniors by helping staff understand and the unique needs of aging individuals and provide training.
+ Encourage ongoing education and awareness among staff about aging-related issues, advancements in geriatric care and best practices
+ Conduct individual and group programming and service delivery workflows that support overall programmatic goals
+ Provide data for analysis to be used for internal reporting for continuous quality improvement, and external reporting for funders
+ Travel to buildings to support program delivery and event logistics , including evening and weekend hours when necessary
+ Perform other duties as assigned
MINIMUM QUALIFICATIONS:
+ LMSW preferred; extensive experience working with the target population may be substituted for a master’s degree
+ Minimum 3 years of experience working with homeless, mentally ill, undocumented, and/or substance using populations
+ Housing-based case management experience strongly preferred
+ Strong clinical and assessment skills preferred
+ Preferred training in CPR and First Aid, Opioid Overdose Prevention, crisis intervention, de-escalation
+ Familiarity with Critical Time Intervention principles preferred
+ Proficiency in motivational interviewing techniques preferred
+ Creativity, strong problem-solving ability
+ Ability to communicate (verbally and written) with diverse populations
+ Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams; proficiency with property management and case management software
+ Ability to travel throughout the 5 boroughs via public transportation or vehicle
+ Valid NY or NJ driver’s license with a good driving record, as defined by Breaking Ground is preferred
+ Bilingual preferred
