Program Coordinator
**Position:** Program Coordinator **Location:** Irondale, AL **Shift:** Monday-Friday; 8am-4pm **Job Description:** To coordinate individual recovery services within the program and the community for individuals in order to increase their inter/intra personal development. Provides case management, group facilitation, and additional supports for individuals and their families. **Essential Duties:** Including but not limited to the following as well as any other duties as assigned by supervisory staff: Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions. Support each person served so that he/she will feel a sense of self determination, security, and have an opportunity to have meaningful relationship with others at home and in the community. Complete daily routine including but not limited to the review of ADP, Dashboard, Progress Notes, Case Workload, and any other documentation. Maintain proper storage and protection of clinical records in accordance with agency policies and procedures and licensing standards. Develop and maintain relationships with outside providers and agencies. Provide individuals with proper referral when needed including but not limited to; an outside psychiatrist, vocational services, or social services. Participate in program assessment and development. Ensure group sessions meet the requirements of regulatory agencies. Complete intake evaluations, necessary assessments, and develop a treatment plan. Provide Individual Therapy, Group Process, Medication Monitoring, and Family Support services. Provide additional services, and complete documentation as needed and as defined by the case workload. Determine treatment services to best meet the individual's needs and assess the client's willingness and readiness to engage in services within the agency. Develop and maintain individual records for each person served on case workload. Plan group sessions and prepare materials to ensure group sessions meet requirements set forth by regulatory bodies. Enter group descriptions in CCApps a week prior to scheduled group. Facilitate group sessions and assist members to integrate and apply what they have learned. Encourage participation in group sessions. Engage individuals who are withdrawn or unwilling to participate. Identify a change or increase in symptoms and intervene as necessary. Ensure that all progress notes and group documentation are completed timely and are in accordance with regulatory bodies. Maintain consistent communication with the agency Psychiatrist. Monitor client’s medications, compliance, and any medication adjustments. Consult with psychiatrist, pharmacy, and other mental health providers, as needed, to maintain compliance with treatment recommendations and medication regimen. Ensure individuals receive and understand their medication by providing education sheets for all psychiatric medications prescribed. Facilitate Interdisciplinary Team Meetings to discuss individual's goals and objectives with both internal and external stakeholders. Identify when an individual is experiencing an increase in symptoms and implement his or her crisis prevention plan. Complete suicide assessment and create a safety plan for individuals who express suicidal ideations. Complete incident reports timely and accurately to ensure compliance with regulatory agencies. Practice and implement all emergency procedures including, but not limited to: fire drills, disaster drills, and other crisis response procedures. Address grievances of persons served and provide appropriate resolution. Respond to and address corrective action measures related to individual record and service provision within agency timeframes. **Qualifications:** To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must be 18 years or older and possess a valid driver’s license. Clearance following a federal and state criminal background check. Clearance based on a 3-year motor vehicle abstract assessment, and annually thereafter. An initial tuberculosis screening;Annual tuberculosis screening to be completed only if there has been known exposure or evidence of ongoing TB transmission. Must be able to successfully complete required training which includes but is not limited to: -American Red Cross Adult CPR and First Aid -Crisis Response Training -Initial and annual training on Preventing Abuse and Neglect and Reporting -Initial Infection Control Training and Universal Precautions -Professional and/or Specialized training as required
