Job Summary:

In this role, you will be responsible for the development and implementation of Individual Support Plans (ISPs), ensuring that all team members are well-trained and equipped to provide the highest quality of care. You will also monitor the health and safety of individuals in residential settings, ensuring compliance with policies and procedures.

Coordinate and participate in the development of the Individual Support Plan (ISP) to ensure measurable outcomes.Supervise and evaluate team members, providing leadership and guidance to foster a collaborative environment.Conduct quarterly Record and Therapy Reviews to ensure documentation is current and complete.Make weekly visits to the ISL to monitor individual status and ensure a safe and clean environment.Maintain client profiles in the database and prepare annual ISL budgets.Work with Primary Support Staff and Residential Nurse to address medical needs and promote wellness.Complete daily documentation audits in Therapy, including health, medications, and daily notes.Facilitate opportunities for natural support and community participation.Other duties as assigned.Education, Experience, and/or Credential Qualifications:
A bachelor’s degree from an accredited college or university.Relevant experience in the field of Developmental Disabilities can be substituted year for year for a degree.Additional Qualifications:
Must have a reliable means of communication.Must possess skills to read and understand written materials.Must possess skills to write clearly and legibly.May be required to assist with physical transfers or lifting, depending on the needed supports of the individual.Physical Requirements:
ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up to 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body.Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
