Program Coordinator - Behavioral Health
SUMMARY OF POSITION Mainstream Living provides services designed to enhance opportunities, create success and fulfill dreams. The Behavioral Health (BH) Program Coordinator position is a professional direct staff position which leads and manages the department toward its primary mission: to partner with persons with a behavioral health diagnosis and their advocates provide opportunities and support which lead to success. This position works to maximize departmental performance through collaboration with other departments for the successful and consistent operation of agency functions and through the direct supervision of Team Leaders (TL’s). This position also provides direct service to the members served in the absence of Direct Support Professional (DSP) staff, and supervises direct support staff activities. This position ensures that programs meet standards defined by the regulating bodies. QUALIFICATIONS AND SKILLS Education: Undergraduate degree is required, preferably in Human Services, psychology, sociology, health care administration, public administration, or be a Registered Nurse in Iowa. Experience: Behavioral Health and/or Human Service experience is strongly preferred. 3 years supervisory experience in any field is required. Driving: This position requires possession of valid Driver’s License, legal driving privileges and ability in the State of Iowa and qualification with Mainstream Living’s underwriter as an insurable driver. Computer: Computer skills including data entry, Word Processing, Google Docs, and basic spreadsheet skills required. Post hire: Must demonstrate competencies to complete internal training, including training to pass medication. The Program Coordinator position requires great attention to detail and superior organizational skills. This position requires excellent problem solving skills with the ability to assess, direct, delegate and follow up to ensure systems are in place for the safe and efficient provision of service. This position requires excellent communication skills with the ability to listen effectively, respond appropriately, and maintain mutual comfort level while relating to diversity. Must have the ability to plan and follow a prescribed budget which requires the utilization of basic and complex mathematical skills. Must have the ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Have the ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form. This individual must be a stable, mature, professional person capable of working in stressful situations. Must possess excellent judgment in crisis situations and demonstrate the same. Must be flexible and able to collaborate with the team to develop strategies and systems that resolve problems. Must be able to objectively examine situations and determine appropriate courses of action. ESSENTIAL DUTIES AND RESPONSIBILITIES This position is considered an essential health care worker position and in the event of an emergency may be required to work one-on-one with members to ensure their safety. Supervisory Responsibilities : Directly supervises Team Leaders and assists with supervision of Assistant Team Leaders and DSP positions as needed/assigned. Member related duties : + Ensure that Mainstream Living policies and procedures and regulatory standards and guidelines are implemented appropriately for each member within their living environment and each employee within their work environment. + Monitor member sites and living environments to ensure safety and implement systems to address safety concerns, trends or hazards. + Assist in the development and monitoring of program and individualized member plans and objectives as needed. + Monitor member site budgets and ensure that service provision meets the requirements of each individual’s plan. + Assist with member crisis intervention and risk management planning. + Coordinate and monitor service authorizations, tier assignments, member eligibility and funding requirements. + Coordinate on-boarding of new member referrals for intake and transfer activities. + Coordinate with member Case Managers, parents, guardians, and others as needed, to facilitate individualized service provision. + Complete serious incident reports for all necessary member related incidents. + Promote members’ health and well-being in a manner which is satisfactory to the member and their IDT and Mainstream values. + Assisting TL’s and medical staff efforts to monitor individual and overall health concerns and oversee safety and cleanliness inspections for all locations where people receive services. + Collaborate with TLs in the areas of member supervision, safety and service provision. + Coordinate with TLs to ensure we adequately address member concerns or needs with input from guardians and relevant stakeholders. + Assist in the development, training, implementation and monitoring of member behavioral support plans. + Conduct member meetings and/or investigations if needed and oversee grievance resolution procedures as necessary to meet the needs of members. + Monitor and address issues relevant to the satisfaction of members, guardians and other relevant stakeholders. Supervisory/Administrative related : + Assist with interviewing, hiring, training, planning, and coordination of the daily work activities of TL’s, and other direct support staff as needed. + Coach, monitor and facilitate the performance and effectiveness of TL’s and ATL’s. Meeting the function of this position in the event of an emergency or staffing shortage. + Evaluate TL’s, and assist with the evaluation and monitoring of direct support professionals’ performance as necessary for discipline and complaint resolution purposes. + Collaborate with Human Resources in activities related to recruitment and retention and employee relations. + Assist in the onboarding process of new employees. Assist in the development, delivery and documentation of member specific training activities. + Conduct investigations and oversee grievance resolution procedures as necessary to meet the needs of members and their direct support staff. + Oversee accurate and timely documentation process for all direct support staff, and submission of required documentation for billing services. This may include: monitoring TL activities in relation to billing requirements; training direct support staff, reviewing member progress notes and following up to ensure that service provision is as defined in the individualized plan and documentation accurately reflects the services provided. + Ensure proper employee feedback and discipline with adequate investigation and consultation with direct care staff, members and others as needed. + Provide any specialized training as required to meet the needs of members. + Conduct staff meetings. + Aid in the strategic plan development and meeting of Strategic Plan outcomes. + Aid in the development, review and revision of policies and protocols. + Adhere to on-call time and respond according to agency procedure and protocol. + Other duties as assigned. Adherence to Policies and Procedures : All employees will adhere to Mainstream Living policies and procedures including but not limited to: + Personnel Policies and Policy manuals. + Attend and work shifts as scheduled. Adhere to Mainstream Living Attendance Policies for time off, on-call and attendance to training/staff meetings. + Follow medication and medical policy and procedures to include: administration, record keeping, identification of medication, purpose, therapeutic intent and adverse reactions, and self-administration of medication. + Follow documentation procedures to include accurate, honest and timely completion. + Follow health and safety practices per Mainstream Living policies including participation in emergency procedures, drills, proper infection control, accident/injury prevention, etc. + Follow vehicle policies for safe operation of Mainstream Living vehicles and safe transport of members/others if in driving position. + Keep supervisor informed of critical and non-routine events. Communication is constructive, effective, and timely. WORK ENVIRONMENT The work environment characteristics described are representative of those an employee encounters while performing the essential functions of this job. Physical Demands: While performing this job, employees must be able to meet the physical demands required to complete essential functions of this position and meet member needs in their home environment. Employees will be required to lift 15-25 pounds on a frequent basis. Depending on member needs and in some work locations, employees may be required to assist with ambulation, lifting and transferring of members, generally with the use of adaptive equipment such as gait belts, walkers, wheelchairs, mechanical lifts, etc. Thus, there may be times the employee must lift up to 50lbs and rarely up to 100lbs if working in sites with members requiring ambulation assistance. OSHA CLASSIFICATION This position is considered a Classification I in regard to jobs in which required tasks routinely involve a potential for mucous membrane or skin contact with blood, body fluids, tissues or potential spills or splashes. Additionally, there is possible exposure to cleaning chemicals. Use of appropriate measures is required for every health care provider.
