II.     Major Responsibilities: 
1.    Assign and monitor key team workflows, including client Government Performance Results Act (GPRA) assessments, Bureau of Substance Abuse Services (BSAS) documentation, 6 month follow ups and ensure timely submission of these forms (including Service Logs). Role is responsible for following up with provider and/or Medical Director to address any non-compliance.
2.    Collaborates with UMMHC grant administrator, medical director, and departmental leadership to ensure compliance with the $600k annual budget supporting 60-80 patients at any given time.
3.    Ensures providers collaborate with outpatient and inpatient multidisciplinary team to enroll patients in a treatment program, with either a PCP specializing in substance abuse or outpatient program. This should occur following approximately 6 months of enrollment in the MDC the program (participation length will vary by patient).
4.    Develops marketing collateral (written materials and workflows) for patient education and community engagement to ensure patients are maximally engaged in leading their care.
5.    Facilitates outreach with various community-based recovery and medical organizations in the Central MA region, partnering with organizations that provide patients with a variety of services. This may include managing subcontracts with community organizations which may provide unique services tailored to the patient’s specific needs.
6.    Liaises with the state to maximize resources for program and patients. 
7.    Develops administrative support mechanisms and communications strategies to grow the MDC program and expend services, in conjunction with program medical director, through collaboration with BSAS; outreach to community programs to develop programs/pathways/etc. with them.
8.    Leads continuous quality improvement, including identifying and prioritizing new QI initiatives and conducting Plan-Do-Study-Act (PDSA) cycles to measurably improve performance. Generates documentation of these and other innovations including in posters, presentations, and written materials.
9.    Develops and conducts training programs regarding recovery-oriented systems of care and managing grants by gathering resources from existing materials/programs/trainings and will adapt them to meet program needs.
10.    Will present feedback and suggestions to improve program to program medical director. 
11.    Establish and publish clear priorities among project activities.
12.    Other duties as assigned.

Preferred:
1.    Grant management and supervisory experience.
2.    Persons with lived experience of addiction.
3.    Experience with trauma informed care.
4.    Experience working with a culturally diverse population.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies and their measurements will be developed and maintained in the individual departments.  The competencies will be maintained and attached to the departmental job description.  Responsible managers will review competencies with position incumbents.
