Serving as the primary point of contact for the program, this role is responsible for ensuring the logistical success of the mobile trauma-informed primary care service. Street medicine is a mobile trauma-informed primary care and outreach service conducted in partnership with several local agencies and with learners involved in the care. Minimum Qualifications Undergraduate degree in a related health, allied health, social science area, or related field and 2 years of relevant experience or a combination of related education and experience. Related experience can include experience gained through intern/externships, volunteering, and/or student employment. At least 1 year experience working or volunteering in a clinical or healthcare setting Experience working with individuals who may be experiencing homelessness, mental illness, substance use disorder, or comparable experience. Current Basic Life Support (BLS) CPR certification with AED training is required at the time of hire. Highly organized individual with excellent oral and written communication skills. Current valid Ohio state driver’s license or ability to travel within the Montgomery County region. Preferred Qualifications Medical Assistant Certification At least one year of experience assisting in a clinical setting. Essential Functions and percent of time: Clinical Responsibilities—40%: Take medical histories and records vital signs, explain treatment procedures to patients, and prepare them for examinations and procedures. Assist primary care providers with patient care. As directed by a Physician, perform and process blood draws, administer medication, and collect specimens. Assist with procedures and change sterile and non-sterile dressings. Interview and accurately record medical and social history, vital signs, and lab results. Administer vaccines and single-dose medications. Document in the electronic medical record and complete other project record keeping. Provide basic health education on various prevention and health maintenance topics, including reproductive health, immunization, and well-child visits. Assist with supply management, including clinical supply stocking, checking expiration dates, etc. Monitor, request, stock, and clean routine clinic supplies; maintain safety and cleanliness of examination/ treatment rooms and laboratory areas. As directed by a Physician, phone in prescriptions to pharmacies, schedule follow-up appointments, and refer patients for specialty and/or follow-up care as needed. Attend training and respond to emergency situations on the unit. Perform pre-visit planning activities and facilitate the pre-visit huddles. In collaboration with leadership and physicians, develop, interpret, advise and implement regulations, policies and procedures. Perform other clinical tasks as assigned. Essential Functions and percent of time (cont'd): Care Coordination and Follow-up – 60%: Coordinate the pre-event scheduling with providers, partners, and learners and takes the lead on patient follow-up and referrals. The coordinator is the thread of continuity in the street medicine team. Coordinate the street medicine team schedule, including direct care providers, the mobile unit, and social service agencies. Collaborate with site staff to ensure the smooth operation of outreach clinics. Promote clinic services at partner sites and ensure staff are equipped with necessary information to advertise service days to clients. Assist in clinic and event setup and provide support on-site during events. Greet patients and partners in a welcoming and professional manner. Answer telephone inquiries related to the street medicine project. Schedule appointments and completes referral scheduling. Provide case management and social service connections. Participate in the quality improvement process and care reflections. Update and maintain patient medical records and assists project directors with reports to funders and others. Participate in outreach events connecting street medicine with community members and agency partners. Perform other administrative duties as directed. Non-Essential Functions and percent of time: Working Conditions The work environment and schedule include at least one clinic half a day per week in a mobile setting. Some evening outreach and education may be required.
