WELLBE INTRODUCTION

The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients.  This population is typically underserved and very challenged with access to care.   To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care.  WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

 

GENERAL SUMMARY

The Project Manager participates in cross functional internal and external teams to define, create, pilot, and launch various projects that benefit the WellBe Senior Medical model.   As the project manager for operations, you will function as a “SME” leading initiative in a fast-paced environment that leads to progressive decision making based on project risk, timelines, budget, and on time deliverables.   This position requires time management, through communication, agility, organizational excellence, reporting, high degree of healthcare knowledge, and finally project effectiveness.

Leads and motivates teams on internal and external projects in a fast-paced start-up environmentFunction as a subject matter expert (SME) leading effective coordination between organization/department process flows, requirements definition, planning, design, rollout and monitoring.Meet with stakeholders to define project scope, business and operational requirements, and produce timelines that align to client and business needsAbility to anticipate issues and work toward a solution by building consensus within the project team.Participates in projects across the organization and be ultimately responsible for the delivery of high-quality deliverables that power client satisfaction, meet business requirements, and balance client economicsInterface heavily with marketing, technical, clinical, patient experience and market leadership to assure that the solution is understood and addresses the customer's business requirementsManage the execution of work, which includes tracking team progress, routing deliverables for approval, monitoring scope, roll out and trainingActively monitor project risks/issues, escalate in a timely manner, and drive teams to effective solutionsDevelop and manage detailed project plan(s) that integrates key activities, timelines, resources, budget, and track the critical path for assigned project(s).Ensure uniform processes/documentation between projects including project meeting organization, issuing agendas, documenting project team meeting minutes, tracking, reporting progress on projects and proactively managing follow up and resolution on action items.Facilitate decision-making process within the team.Foster open dialogue within team and build positive team spirit to overcome obstacles.Support contractual and intellectual property requirementsOther tasks needed to accomplish team’s objectives/goals Job Requirements

QUALIFICATIONS

 

Educational Requirements:

BS/BA in relevant program, or equivalent work experience5+ years of project management experience 2+ years’ experience in healthcare 3+ years applicable experience as a Project Manager Solid understanding of project management methodology, including the ability to identify and resolve issues, manage risk, and develop detailed work plans and specifications

 

Required Skills and Abilities:

Experience working as a project manager on external client facing projectsHealthcare experience is requiredExperience in start-up or fast paced dynamic environmentFamiliarity with Agile MethodologyAbility to multi-task and manage competing prioritiesExperience interfacing with clients and partner vendors on various project deliverablesMust be able to work in a matrix environment Ability to build strong multidisciplinary cross-functional teams and influence othersMust have the ability to motivate others, prioritize multiple tasks, and maintain positive interpersonal working relationshipsExcellent written and verbal communication skills are required. Must be able to communicate effectively with all levels of the organizationExcellent understanding and proficiency with Microsoft Office including strong MS Project skills and other project management software and standard Project Management tools and methodologiesPMP certification desirable

 

Supervisory Responsibility: No supervisory responsibilities.

 

Travel requirements: Local to National travel up to 10% of the time. This may include extended lengths of time away from home base. 

 

Work Conditions: Ability to lift up to 20lbs.  Moving lifting or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs.
