HOURS: 8:30am-4:30pm or 9:00am-5:00pm
Credential/License/Education: Minimum Master's Degree in related field: MSW, MFT, MHC, MSW
Rate of Pay: $$62,400 (LP) to $65,000 (LMSW, LMHC, LMFT, LCSW)
JOB DESCRIPTION:
The Guidance Center of Westchester Personalized Recovery-Oriented Services (PROS) program seeks a dynamic and motivated individual that has a passion to serve the underserved mental health population, while learning and growing in thier professional career for the position of PROS Recovery Counselor, at our main Mount Vernon NY location, walking distance to Metro North. PROS offers adults with serious mental illness a broad range of integrated services designed to help them achieve goals for work, education, relationships, socialization, living, and health and wellness.
SKILLS/ABILITY:
Capacity to provide recovery based rehabilitative approaches with adults with psychiatric disabilities and associated functional impairments.
Knowledge of evidence-based practices.
Capacity to work with multi-disciplinary team
Ability to plan, develop and run curriculum-based classes and groups.
Computer proficiency including knowledge of electronic health reporting.
Ability to satisfactorily complete required documentation with regard to due dates and content.
Strong oral and written communication skills.
Must be able to use and train staff to use a hybrid. model of telehealth and in person service delivery and staff meetings.
Specific Responsibilities:
Provide recovery-based service coordination for caseload of 18-20 (on average) participants with psychiatric disabilities and secondary learning or substance use diagnoses.
Collaborate with caseload participants to develop individualized recovery plans and complete monthly progress reports within required time frames
Schedule and hold weekly meetings with each caseload participant to assess progress, plan, schedule, and/or provide clinical service interventions.
Complete all required EHR and paper documentation required in the case record
Conduct required psychosocial, substance use screenings, psychiatric rehabilitation, and other assessments within required time frames.
Develop syllabi and lesson plans for, and facilitate/teach PROS classes or groups (average 10-12 per week) consistent with PROS services
Monitor attendance of caseload participants to notice unexplained absences and to reach out/ intervene in a timely manner
Attend and participate actively in weekly staff meetings and other agency or program meetings as required.
Participate in case presentations and conferences with other staff, family members of participants, other service providers, etc.
Cover classes and individual services for absent staff as requested by supervisor.
Provide crisis intervention services for participants.
Initiate, maintain and document contact with outside mental health, medical, housing and social services service providers of caseload participants.
Participate in in-service trainings, workshops, webinars, and conferences as requested by supervisor
Work cooperatively and collaboratively with a multi-disciplinary team to promote and sustain best practices
May participate in training non-professional clinical staff
Any other duties as assigned.
