**Responsibilities:**
GENERAL PURPOSE:
The Department of Psychiatry at the University of Rochester invites applications for a Care Manager position to join the Collaborative Care and Wellness Division. Specifically, Behavioral Health Partners (BHP) and University Mental Health Practice (UMHP). BHP is a mental health resource for the University of Rochester eligible employees, retirees, and adult dependents enrolled in a University Health Care Plan. University Mental Health Practice (UMHP) provides outpatient psychological and psychiatric services for individuals aged 14 and older who require specialized treatments. The Care Manager is an integral member of the BHP and UMHP teams. The Care Manager is responsible for the managing the eRecord workqueue, assisting clinician’s with managing their case lists, managing resource lists, developing workflows and other duties as assigned.
**ESSENTIAL FUNCTIONS**
**Incoming Referrals:**
+ Triage UMHP and BHP referrals needing clinical review in workqueues
+ Contacts referring providers for clarification or to coordinate communication for incoming referrals
**Documentation/Paperwork:**
+ Assists in completing referral paperwork for outgoing referrals
+ Support all UMHP and BHP providers with three-week letters etc.
+ Manage paperwork related to case management
**Care Collaboration:**
+ Help identify resources for current clients
+ Update resources lists for all providers
+ Manage BHP-AHP collaboration and referrals
+ Collaborating with team members
+ Collaborating with treatment team providers within and external to URMC.
**Case management:**
+ Track high risk clients
+ Triage incoming crisis call/communications
+ Manage 45 days lists and collaborate with providers around next steps
**Client Support:**
+ Provide direct care management services to clients as directed by clinical staff to support client in obtaining treatment goals.
**Process Development:**
+ With direct oversight from leadership, carry out actionable items for quality improvement and process development
**Other:**
+ Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competence events and training appropriate to job duties.

**QUALIFICATIONS**
+ Associate’s Degree in health-related field and previous experience in human services (preferably in a psychiatric setting) required.
+ or an equivalent combination of education and experience preferred.
+ Demonstrated customer service skills required.
+ Detail oriented with strong organizational/prioritization skills and ability to work independently under general guidance required.
+ Ambulatory care and telephone triage experience preferred.
+ DL NUMBER - Driver License, Valid and in State Must possess a valid New York State Driver's License and have access to automobile upon hire required.
