SCOPE OF ROLE:
The ACT teams Psychiatric Nurse Practitioner partners with the multi-disciplinary team to deliver comprehensive mental health treatment to individuals who have a mental illness that seriously impairs their functioning in the community. Serving those whose needs have not been met by traditional clinic-based approaches, this innovative, recovery-oriented, ‘best practice’ intervention delivers treatment to people in their homes, shelters, parks and anywhere else they happen to be living, engaging them in a sustained relationship that leads to recovery. ACT Teams are a mobile interdisciplinary team of professionals (psychiatry, nursing, social work, rehabilitation).
ESSENTIAL DUTIES & RESPONSIBILITIES:
Write prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements.Review health assessments completed by the Program Nurse and provide clinical feedback.Provide medication management including prescription refills and medication education to consumers.Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes.Provide training to other team members on topics including medications and drug interactions.Communicate and coordinate care with psychiatrists in the community, as needed.Assist with psychiatric emergencies, refer residents to emergency care if needed.Provide crisis intervention and address all emergency situations as appropriate.Maintain up to date, accurate, complete, and timely consumer chart documentation.Attend staff meetings, case conferences, group supervision, and trainings as needed.Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.Coordinates care with S:US nurse practitioners, medical director and administrative staff; closely coordinates with clinicians external to S:US as appropriate.Utilizes best and evidence-based approaches to treatment consistent with S:US' organizational culture; health/mental health integration, rehabilitation and recovery, and wellness approaches figure prominently.Perform other related duties as assigned.

QualificationsREQUIRED EDUCATION AND EXPERIENCE
Licensed as a Registered Nurse in New York State and credentialed by the State to practice as an advanced Nurse Practitioner with a specialty in psychiatry. Must be registered as a NY State Medicaid provider. Must have a DEA# and an individual NPI #.Maintain CPR/First Aid Certification.

Preference is given to those who have experience working on a multi- disciplinary team and/or previous experience with severely and persistently mentally ill, substance use and homeless populations.
