General Summary:
In collaboration with Psychiatric Urgent Care (PUC) clinical providers, coordinates linkages to care for urgent care patients. Patients include children, adolescents, adults and older adults. Data collection and patient registration. Coordinates with Admission, Day Hospital, Care Connect and other programs. Communicates with patients/families and with clinical providers. Functions as liaison between community referral sources and Psychiatric Urgent Care providers.Knowledge, Skills, and Abilities Required:
Work requires knowledge of psychiatric disorders and diagnosis as acquired through a Bachelor’s degree in Psychology, Social Work, Psychiatric Rehabilitation or related field of study. For internal candidates, high school diploma plus an additional 3 year of relevant internal experience may be considered as a substitute for a bachelor's degree. Position requires computer skills and ability to learn new computer programs. Work requires interpersonal skills to communicate effectively with patients, families, referral sources and co-workers, sometimes under stressful circumstances (51-80% of work time). Urgent Care patients and their families may be stressed, anxious, irritable and/or distraught.
