Responsibilities
Pathways Center ’s Community-based rehabilitation services are designed to help adult persons with psychiatric disabilities/disorders or other behavioral health needs to optimize their personal, social, and vocational competencies in order to live successfully in the community. Participation is determined by the needs of the person served and the person served is an active partner in all aspects of programming.
This direct care position works directly with adult mental health consumers in a Psychosocial Rehabilitation (PSR) program that provides therapeutic, rehabilitative, skill building, and recovery services for individuals to gain the skills necessary to allow them to remain in or return to naturally occurring community settings and activities.
All programs have designated space, transportation and resources available. Program hours are generally Monday through Friday from 9am until 2pm, with varied weekend and evening hours.
The Case Manager/Social Service Technician III is a direct care provider whose responsibilities include but are not limited to the following:
□ Individual or group skill building activities that focuses on the development of skills to be used by individuals in their living, learning, social and working environments,
□ Social, problem solving and coping skill development;
□ Illness and medication self-management;
□ Participates in Team Staff Meetings; Co-ordination of services within the agency
□ Actively engages internal and external stakeholders to recruit new referrals.
□ Transports clients as needed to and from the program
□ Leads recreational and leisure activities that improve self-esteem and recovery
□ Attends clinical supervision and professional development opportunities as applicable
□ Documentation within required time frames to reflect interventions and the consumers progress toward his/her goals; Treatment plan development
□ Assists clients with linkage to community resources and other case management needs.
□ Assist the CPRP or CPS with the implementation of evidenced based practices
□ Prevocational skills (for example: preparing for the workday; appropriate work attire and personal presentation including hygiene and use of personal effects such as makeup, jewelry, perfume/cologne etc as appropriate to the work environment; time management; prioritizing tasks; taking direction from supervisors; appropriate use of break times and sick/personal leave; importance of learning and following the policies/rules and procedures of the workplace; workplace safety; problem solving/conflict resolution in the workplace; communication and relationships with coworkers and supervisors; resume and job application development; on-task behavior and task completion skills such as avoiding distraction from work tasks, following a task through to completion, asking for help when needed, making sure deadlines are clarified and adhered to, etc; learning common work tasks or daily living tasks likely to be utilized in the workplace such as telephone skills, food preparation, organizing/filing, scheduling/participating in/leading meetings, computer skills etc) and
Recreational activities/leisure skills that improve self-esteem and recovery
Minimum Qualifications
Bachelor’s degree in a social services related field and one year experience in a social services related position. A valid driver’s license with a clean driving record. Candidates with an interest in pursuing a Certified Psychosocial Rehabilitation Provider (CPRP) will be strongly considered for this position. Must have experience in clinical documentation.
High school diploma/GED and three (3) years of job-related experience in a social services-related position; or one (1) year of experience at the lower level Social Svcs Tech 2 (SST011) or equivalent position.
