Location: Washington - 239 Hurffville-CrossKeys Road Employment Type: Employee Employment Classification: Regular Time Type: Full time Work Shift: 1st Shift (United States of America) Total Weekly Hours: 40 Additional Locations: Linwood - 1201 New Road, Suite 150 Job Information: The Licensed Clinical Social Worker will provide both clinical and psychotherapy services in a fast-paced primary care setting. Therapists will be working with medically complex patients who are in need of mental health counseling and medication management. Approximately 8-12 sessions will be provided to patients. Therapists will collaborate with treatment team to provide patients with therapeutic services including counseling, medication management collaborative services, and connection to community services and or resources as needed. The candidate must be comfortable working with diverse range of patients in terms of age (18 and older), race, culture/ethnicity, gender, sexual orientation, religion, income and educational levels with medical and mental health needs. Position Responsibilities: Provide effective individual, family and group psychotherapy; determine realistic goals; manages appropriate referrals and resources within Virtua Medical Group to support physician follow-up and patient engagement. Responsible for outreach efforts to establish and maintain positive working relationships with our key customers (physicians, office staff, patients and external customers). Communicates effectively with Primary Care Providers; and collaborate with patient and care team as needed regarding medication changes and management Attend program development for patients treated or evaluated; demonstrate an understanding of patient needs when devising and implementing treatment; provide team consultation; Appropriate and comprehensive complete biopsychosocial assessments, patient centered individual plan of care including treatment goals; document updates of treatment goals, and document individual and family sessions. Meet with patients for 6-12 sessions to reinforce goals during short term therapy sessions; crisis intervention as needed. Metrics- Accountable to job specific goals, objectives, and dashboards, which contributes to the success of the organization. Compliance - Understands and applies applicable federal and state regulatory requirements. Participates in organizational improvement activities, including patient satisfaction team, six sigma teams, department/divisional teams and community events. Position Qualifications required: Required Experience: At least 3 years’ experience as a LCSW in a community mental health setting, medical center or other related field preferred. Experience working in behavioral medicine and psychology setting. Experience working as a member of an interdisciplinary team and ability to communicate and collaborate effectively with a variety of professional, ancillary and administrative personnel. Must have medical literacy. Must be familiar with Cognitive Behavioral, Solution focused, and Motivational Interviewing techniques. Must be able to complete biopsychosocial assessments and assist patients with defining goals for treatment and effectively use a DSM V to diagnose and provide patient education. Must be familiar with community treatment options and or other resources to refer and connect patient to as needed. Must possess good computer skills and be familiar with using and documenting in EMR system. Must have strong interpersonal, communication, and organizational skills. Required Education: Master’s Degree in Social Work (MSW) is required from an accredited university or college. Training / Certification / Licensures: Current State of New Jersey Social Work License, Licensed Clinical Social Work (LCSW) required
