For more than 30 years, Community Home Health has delivered excellent home care to patients and families in Fresno County and surrounding areas – helping them make the transition from hospital to home.
As a part of Community Regional Medical Center, we are one of the Valley’s largest and most comprehensive programs, offering a full continuum of care, including Home Health and Home Infusion services. Home care provides a welcome alternative to frequent and prolonged hospital stays.
Responsibilities:In this role some of your key responsibilities would include but are not limited to:
Evaluating, planning, organizing, and implementing specific treatment programs for individual patients. Implements therapy according to established plan, utilizing appropriate procedures, equipment and techniques within the scope of physical therapy.Educates the patient and/or the caregiver about the findings of the evaluation, established goals of the physical therapy program, plan of treatment, home programs, injury prevention, and other related information. Communicates effectively with other patient care team members and coordinates care provided. Supervises PT Assistants with implementation of treatment plan.Qualifications:Minimum requirements
• Bachelor's Degree in Physical Therapy
State of California Physical Therapist License or eligible for licensure
• Valid California Driver's License and driving record acceptable to Community Medical Centers required
• Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA)
Preferred qualifications:
1 year of experience in an inpatient or outpatient clinic settingHave questions?
