Public Health Nurse

Position Summary

Provides oversight of the daily management of the Community Health Program. Provides a full range of public health nursing services to the community of Pueblo of Sandia (PS).  Services are provided in patients’ homes, the health clinic, senior meal site, and community center and are provided to all age groups from pre-natal through senior citizens.

 Essential Duties And Responsibilities

Plans, conducts and evaluates a comprehensive community health nursing program for the Pueblo of Sandia community to align with the PSHC mission statement. Assesses and diagnoses needs and sets up a plan of action to meet these needs by providing health services to families and individuals. Provides primary care for acute and chronic health problems in collaboration with medical clinic staff and homecare programs. Directs all activities of medical and safety Community Health Representatives (CHRs) and van driver MTD, including organization and documentation of their activities. Plans and participates in Programs for Diabetes Management and Primary Prevention. Plans and participates in Programs for Infectious Disease Control. Provides in-home healthcare to all populations from pre-natal care to geriatric care. Provides skilled nursing services to individuals and families, including measures to promote wellness, to prevent complications and to minimize disabilities. Teaches recognition of early symptoms of communicable diseases and the importance of immunizations. Updates immunization records and establishes a notification to patients that immunizations are required. Gives immunizations to patients who come into the clinic specifically for their vaccines and/or immunizations as prescribed by the Public Health Nurse PHN. Records all immunizations given in the clinic onto the NMIIS statewide system as appropriate. Conducts epidemiological and community surveys, field investigations and research studies to appraise health needs. Participates in continuing quality improvement activities for the Public Health Nursing program. Places referrals for routine care: mammography, colonoscopies, referral renewals, etc. suggestion Maintains records and reporting systems according to Sandia Health Center PSHC policies for patients. Works with the Tribe, State and Federal Health Agencies, in planning health related activities. Punctual and regular attendance is an essential responsibility for all team members and are expected to report to work based upon the assigned schedule each day in accordance with the Attendance Policy for Pueblo of Sandia.  

 Additional Responsibilities

May be crossed trained and act as covering PSHC Registered Nurse, or medical and/or safety CHR. Demonstrates strong interpersonal skills in building a cohesive working relationship with co-workers and internal/external contact. Performs other job-related duties as assigned.

This position has Key Performance Indicators (KPI’s) identified as a measurement of success.  KPI’s will be shared with all team members who work in this job title and feedback will be provided regarding the successful completion of the KPI’s as part of the performance review process.

Must maintain a high degree of confidentiality via demonstrating the ability to respect and protect patient information in accordance with HIPAA law(s). Ability to read and write adequately to complete required documentation and interpret all written communication from PSHC supervisors or administrators. Must have good written and oral communication skills with a demonstrated ability to prepare and deliver oral and written reports. Ability to travel and attend meetings, conferences and training seminars when applicable. Ability to render basic First Aid and CPR and take vital signs and enter data accurately. Ability to learn new skills and information as needed. Improve patient safety through knowledge of safety regulations, standards and practices. Ability to recognize abnormal symptoms and/or identify problems in adults and children and report them to a provider on behalf of the patient as soon as possible. Ability to advocate for the patient during medical and non-medical appointments. Ability to work a flexible schedule with days, evenings, nights, weekends and holidays as needed. Knowledge of the community or population to be served preferable or extensive multi-cultural experience. Ability to provide services independently without constant observation and supervision. Must be able to deal with the possibility of being subject to inappropriate remarks or actions from clients.  Must have the ability to establish appropriate boundaries regarding behaviors of the client while at the same time being sensitive to, and accepting of, potentially awkward behaviors brought on by the medical condition of the client. Skill in relating to patients and other staff. Ability to interact effectively with the client being served, the community, as well as with PHN and all PSHC staff.    Education And Experience  

 Required

High School Diploma, GED certification or equivalent. Associates degree (BSN) in nursing from an accredited college or university. Three (3) years’ experience in public nursing including two (2) years supervisory experience.

Note:  Relevant work experience or education may be substituted to satisfy education and/or work experience. 

License/Certifications/Registrations

Current RN Licensure with the NM Board of Nursing. Current CPR and First Aid certifications The following immunizations and titers are required prior to employment start date: Hepatitis B series or immune by titer Tdap MMR or immune by titer Annual TB test Annual Flu vaccination (seasonal) COVID booster (optional but encouraged)

List Working Conditions Required:

Work hours subject to change with short notice with possible overtime requirements; Director approval is required. Work is performed primarily indoors, with occasional outdoor exposure when traveling. Environment involves some exposure to physical risks which require following basic safety procedures. Work environment involves some exposure to contagious diseases and or blood borne pathogens and may require use of personal protective equipment. Subject to hazards which may cause personal bodily harm; smoke; diseases; cuts bruises, burns, common cold, influenza, dust, odors and elevated noise levels. Tasks may be performed on uneven, inclined, hard and soft carpeted floors, cement structures and surfaces.
