San Mateo County Health - Family Health Services is seeking a Public Health Nurse (PHN) to support any of the Maternal Child Health Programs, including Nurse-Family Partnership (NFP). NFP is an evidence-based home visiting program that has been demonstrated to significantly improve critical outcomes for at-risk first-time pregnant parents and their children. The three major goals of the program are to (a) improve prenatal outcomes, (b) improve child health and development, and (c) improve self-sufficiency of the family.
This Nurse-Family Partnership nurse home visitor is responsible for providing comprehensive nursing services to first-time moms and their families. The nurse home visitor is responsible for maintaining the highest standards in clinical nursing practice and adherence to the Nurse-Family Partnership model, policies, procedures, Visit-to-Visit Guidelines, and standards set by Family Health Services Division. The nurse home visitor reports to the Nurse-Family Partnership nurse supervisor.
While the primary assignment will be in Nurse-Family Partnership, the successful hire may be required to support other Family Health Services programs and activities based on departmental needs.
The current vacancies are two Full-Time, Regular positions.
The ideal candidate would possess the following:
+ Bachelor of Science in Nursing is preferred.
+ Current CPR certification
+ Proven ability to establish interpersonal relationships with individuals and families from diverse socioeconomic and cultural backgrounds.
+ Excellent written and verbal communication skills.
+ Basic computer skills, electronic health record experience preferred.
+ High degree of autonomy. Ability to impose structure throughout the workday both in between and during visits.
+ Understands, supports, and coaches others in the Nurse-Family Partnership vision, mission, and model.
+ Assists in creating a positive work environment that promotes mentoring, teamwork, and collaboration.
+ Promotes ongoing learning of current nursing practices and innovations in team environment.
+ Ability to speak, read, and write in Spanish is required.
Responsibilities of Public Health Nurses of Nurse-Family Partnership include:
+ Provide home visits and virtual visits to eligible first-time moms from early pregnancy through the child’s second birthday.
+ Carry a case load of 25 or more clients to include:
+ Bi-weekly home visits,
+ Case management and care coordination as needed (the goal is to coach clients to confidently advocate for themselves and their child over time),
+ Promotion of positive maternal health practices through nursing education,
+ Support of Parent/Child dyad through use of child development curriculum and activities,
+ Incorporates a strengths-based approach to develop therapeutic relationships with clients and families,
+ Uses motivational interviewing concepts to support behavioral change,
+ Timely documentation in multiple EHR systems for nursing practice and data collection for evidence based programming and ongoing evaluation.
+ Participate in weekly team meetings and case conferences as well as weekly “Reflective Supervision” through 1:1 meetings with NFP Nurse Supervisor.
+ Adhere to nursing process as indicated by the CA Board of Registered Nursing and the Nurse-Family Partnership model of home visitation.
+ Perform other duties as assigned.

Qualifications
Education and Experience:
+ Any combination of education and experience that would likely provide the required knowledge, skills, and abilities is qualifying.
Licensure/Certification:
+ California license as a Registered Nurse.
+ California Certificate as a Public Health Nurse.
