Qualified Intellectual Disability Professional (QIDP)


Reports to Lead QIPD/ ICF Director
Designs, integrates, coordinates and monitors, all components of the
active treatment effort, including IDT, creates relevant Individual
Support Plans designed to meet the need of each individual client
assigned.


Responsibilities include: Routinely observes clients in program area
settings to assess effectiveness of program implementation and
consistency of training effort. Determines program planning
effectiveness and makes timely modifications facilitating achievement
of desired goals.
Development of homes and ISP for individuals in assigned homes.
Collaborating and working closely with Home Coordinators, Therapists,
Nursing and DSP staff. Ensuring that optimal services are provided in a
manner consistent with SRVS values, mission, and principles, as well as
in accordance with policies, procedures, and Licensing standards.


Qualifications:
Must have Bachelor’s Degree in a related field.
At least one year previous experience working with individuals with
intellectual developmental disabilities.
Knowledge of ICF/IDD standards and licensure policies and regulations.
Effective communication skills, both written and verbal.
Computer and data analysis skills.
Problem solving and conflict resolution skills.
Essential Job Functions:
1. Develop ISP, CFA, IDT meeting goals, monthly reviews, and other
written documents and plans.
2. Provide education/observation on Active Treatment Loop.
3. Writes Individual Support Plans for individuals in assigned homes.
4. Leads Interdisciplinary Treatment Team (IDT) for assigned homes.
5. Responsible for day-to-day program plans for assigned homes.
6. Conducts visits, surveys, and observations assigned by director;
submit documentation as scheduled.
7. Ensure that all demographic information and charts are up to date
and all diagnosis are current and accurate.
8. Maintain effective communication with families, conservators,
and natural supports of person(s) supported.
9. Trained on all individual plans as trainer.
10. Works with RN Area Manager monitoring individual services,
such as appointments, changes, new diagnosis, hospitalizations, etc.
Ensuring health, safety, and continuous improvement.
11. Develops and oversees Community Participation Monthly
Calendar with the Home Coordinator and monitors to ensure
activities are individualized according to ISP.
12. Acts as a liaison with community resources and coordinates
services of consultants and professional services needed by
person(s) supported.
13. Works closely with ICF Director to ensure compliance, agency,
state, CMS polices, regulatory procedures and standards for all
ICF/IDD homes.
14. Other duties assigned.

 

Hours: 8a-5p (Hours could vary)

Salary: $43,000
