QIDP Coordinator Type of Position Full time Search Location(s) Fishkill, NY Apply Now (https://phe.tbe.taleo.net/phe03/ats/careers/v2/applyRequisition?org=CPOFNYS&cws=45&rid=6336) Job Brief As part of a team, the employee in this position is responsible for ensuring that all service plans are created, monitored, and updated in accordance with OPWDD regulations and best practices. Shift Worked: M - F, 9a - 5p Weekly Hours: 37.5 FLSA Status: Exempt Pay range: 58,500 - 60,000 POSITION DUTIES This position is also responsible for ensuring that all staff are trained in the development and implementation of all plans as well as all necessary documentation. The employee in this position will work closely with the Director of Day Services, The Billing Department, Day Habilitation Coordinators, Habilitation Specialist, and Habilitation Aides to ensure that services are provided as identified and specified in the Habilitation Plans. As part of a team, an employee in this position is responsible for assisting individuals to successfully work towards independence, participate in the local community, and promote the individuals’ integration into the community. In a manner consistent with the agency mission and applicable laws and regulations, an employee in this position shall: + Ensure progress toward achieving the values of The Day Habilitation Purpose Statement: “Encouraging people to explore, discover and realize their full potential by fostering connections through shared interests while celebrating what makes us unique,” as well as values of the HCBS Medicaid Waiver, i.e., Independence, Integration into the Community, Individuality, and Productivity. Ensure documentation reflects progress toward achieving higher levels of these values. + In line with CPU’s commitment to CARF Accreditation, the employee will demonstrate a commitment to continually enhance the quality of services and programs, focusing on the satisfaction of the persons served. + Oversee the scheduling and coordination of quarterly, annual, and special review meetings. + Assists with creating and monitoring Staff Action Plans and Day Hab Plans. This includes training staff on how to write and maintain Habilitation Plans. + Review and certify Staff Action Plans and Day Hab Plans. + Ensure that all necessary documentation is gathered upon admission and updated annually, semiannually, or as needed (safeguards, self-med assessments, physicals, PONS, Behavior Plans, Medication Monitoring Plans, etc.). + Assist with the upkeep of documentation in CHOICES. + Ensure that all staff are trained on all new plans whenever changes are made. + Maintains records of staff training on all plans (ensure signatures on Precision Care). + Ensure that all goals are appropriate and consistent with Lifeplans/CFAs. + Ensure that any needed changes in a person’s safeguards or goals are addressed promptly, communicated to the care coordinator, and documented in all appropriate plans. + Assists with training staff on Precision Care use and quality goal writing. + Assists with ensuring that daily goals are documented in a timely manner. + Assists with ensuring that monthly notes are completed in a timely manner and contain all needed information. + Assists with ensuring that daily attendance is documented in a timely and accurate manner. + Monitors all plans and ensures necessary revisions are made. + Conducts staff evaluations and observations. + Provides direct feedback and training to DSPs regularly and as needed. + Will notify the Program Director of any safety concerns immediately upon becoming aware of them. + May conduct and/or participate in fire and evacuation drills. + Provides programmatic training to Hab Specialists in the implementation of goals. + May assume additional supervisory and administrative responsibilities in the absence of the Program Director, including but not limited to: + Ensuring the appropriate utilization of staff. + Ensuring quality assurance and compliance with governmental regulations and agency policy and procedures. + Ensuring that required documentation is completed, i.e., attendance, incident reports, etc. · May be assigned as a member of a standing committee. · Ensures that all Agreements of Services are uploaded into Precision Care. · Completes and responds to all requests for information, including Incident Reports. · Attends and/or conducts services and specialized training programs as assigned. · Demonstrates legal and ethical conduct. · All actions should adhere to the Agency Employee Handbook or Employee Updates. · Perform all other duties assigned. Essential Skills and Experience · Ability to physically assist individuals as required and intervene physically when appropriate using approved techniques to safeguard the individual, oneself, and others during behavioral crises. · Ability to relate appropriately to individuals, their families, and their advocates; and to constructively relate to and communicate with peers and supervisors, and to be able to function successfully as part of a team. · Ability to work independently · Obtain AMAP certification within 3 months of hire · Obtain agency driver authorization within 3 months of hire · Ability to use sound judgment while following agency policies, practices, and procedures. · Successfully complete all agency-required training and keep required training status current on an ongoing basis. · Knowledge of community resources. · Possess computer proficiency, particularly with Microsoft Office. QUALIFICATIONS · BA or BS in an appropriate field of study. Minimum of 5 years of experience working in the field of Developmental Disabilities. QIDP qualified. · Excellent verbal and written communication skills · Proficiency in computer skills · Excellent organizational skills · The employee is required to successfully complete all required trainings.
