The Mobile Outreach Response Engagement Stabilization (MORES) Team Qualified Professional operates under the supervision of the MORES Team Leader to provide therapeutic interventions and coordination of treatment services for each family in care.
Job Duties:
Participate in the person-centered MORES planning (MP) process.
Assist with implementing a home-based crisis evaluation, stabilization and support plan with the youth and his/her caregiver(s) as indicated in the MP.
Provide psycho education as indicated in the MP.
Assist the Team Leader in monitoring and evaluating the effectiveness in interventions, as evidenced by symptom reduction and progress toward goals identified in the MP.
Assist with crisis interventions as necessary.
Assist the Team Leader in consulting with identified providers, engaging community and natural supports, and including their input in the MORES planning process.
Ensure that all documentation is accurately completed in a timely manner that meets state requirements. Documentation includes but is not limited to MCP, service notes, assessments, service authorization requests, and discharge planning/summary.
Qualifications:
Bachelor's degree in related field required.
Experience with acute emotionally challenged children or adolescents required
Must complete all state required training within 90 days of employment, or provide documentation that training requirements have been previously met.
Valid driver's license and First Aid/CPR certifications required.
Must possess basic personal computer knowledge and skills.
Must be 21 years of age.
