As a Qualified Behavioral Health Provider (QBHP), you will help with achieving patient wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation. 

 

Position Highlights & Primary Responsibilities:

Coordination of services both within and outside of Life Strategies Counseling (i.e. counseling, medication management, welfare, vocational services) Must achieve at least 60% Level of Care on individual and collateral visits  Must conduct home visits at least once per month with client and guardian Provide transportation as needed for clients

Required Qualifications:

Valid driver’s license  Ability to travel independently 

 

Preferred Qualifications:

1+ years of related experience and/or training  Proven solid communication skills Ability to coordinate resources
