Overview

QUALIFICATIONS: A minimum of one year of experience in treatment of youth with DD/ID and is one of the following:
• A social worker with a bachelor’s or master’s degree in social work from an accredited program
• A bachelor’s degree or master’s degree in a human services field other than social work and at least three years of social work experience under the supervision of a qualified social worker or at least one year of experience working directly with people with intellectual disabilities or other developmental disabilities.
• A bachelor’s degree in education from an accredited program
• A master’s degree in psychology from an accredited program


SUPERVISION: This position reports to the Director of Case Coordination, who in turn reports to the Executive Director.


POSITION PHILOSOPHY: A Qualified Developmental Disabilities Professional (QDDP) ensures service design and delivery which provides each child with an appropriate active treatment program. This includes ensuring each child receives a comprehensive, person-centered treatment program of support. The QDDP will ensure that any discrepancies or conflicts between programmatic, medical, dietary and vocational aspects of the child’s assessment and program are resolved and follow up recommendations for services, equipment or other programs are initiated. The QDDP is the central communicator surrounding care while at T. C. Harris school and collaborates with stakeholders in the design and implementation of the treatment program and ensures that each individual program plan (IPP/ITP) is consistently implemented and documented.


ESSENTIAL FUNCTIONS LISTED BUT NOT LIMITED TO:
• Prepares, develops, and reviews the student’s overall treatment and support plan. Completes Functional Life Skills assessments, including but not limited to the Ansell Casey, Life Skills Assessment and Sensory Integration Inventory-Revised. Analyzes progress and data to support continued appropriateness/relevance of the Individualized Program Plan (IPP/ITP). Provides ongoing training to staff on plan implementation.
• Develops program area schedules, routine, interactive materials, and outings that support the growth and development of each student in the program area. Provides guidance on the floor in the milieu to ensure proper implementation. Collaborates with the program department staff to ensure the routine is followed and consistency is delivered to students.
• Establishes and develops Individualized Program Plans (IPP/ITP), Independent Living goals and support strategies based on information gathered on the Functional Life Skills Assessment, Ansell-Casey, internal and external team members. Monitors the implementation of independent living goals and strategies. Collaborates with program staff to ensure that each student’s plan is consistently implemented and documented. Amends IPP as needed to address student’s deficits.
• Prepares progress reports on the Individualized Program Plan (IPP/ITP), quarterly and monthly reports, completes NYTD reports, and progressing on student IEP’s.
• Schedules, coordinates, facilitates, and participates in meetings (IEP, TTR, Team meetings, etc.) with the interdisciplinary treatment team identifying areas of student success, where student may need additional supports and identifying the most appropriate methods for addressing their needs. Attends staffing with the psychiatrist.
