Quality Assurance Investigator

Our Position: We are currently seeking a Quality Assurance Investigator to conduct the investigation of allegations of abuse/neglect and other reportable incidents as prescribed in OPWDD Part 624 regulations. This position also involves: + Ensures that reports are generated in appropriate format, including recommendations to address the situation as well as any systemic issues, responsible for incident management and working in the OPWDD Incident Reporting Management Application and Justice Center Vulnerable Persons Central Register. + Maintains open channels of communication with program staff and support staff to ensure a positive team approach to incident management. + Recommends policy/procedural changes with respect to issues that are raised during the course of investigations. Our Rate of Pay: $ 24.00- $27.00 Our Requirements: + Master’s Degree in a Human Services or related field PLUS less than one year’s experience in human services. OR + Bachelor’s Degree in a Human Services or related field PLUS one years’ experience in human services. OR + Associate degree in a Human Services related field PLUS two years’ experience in human services. OR + High School Diploma or equivalent diploma PLUS three years’ experience in human services. See our full Job Description listed below: Job Description Job Title: Quality Improvement Investigator Department: Quality Improvement and Corporate Compliance Job Grade: 4 Position Summary: Conducts the investigation of allegations of abuse/neglect and other reportable incidents as prescribed in OPWDD Part 624 regulations. Reporting Relationship: Works under supervision of the Director of Quality Improvement and Corporate Compliance. Job Duties and Responsibilities: + Assumes primary responsibility for the investigation of allegations of abuse/neglect and other reportable incidents. + Ensures that reports are generated in appropriate format, including recommendations to address the situation as well as any systemic issues, responsible for incident management and working in the OPWDD Incident Reporting Management Application and Justice Center Vulnerable Persons Central Register. + Ensures compliance with all incident investigation and reporting requirements per OPWDD Part 624 regulations. + Recommends policy/procedural changes with respect to issues that are raised during the course of investigations. + Assists in the provision of staff training and development. + Maintains open channels of communication with program staff and support staff to ensure a positive team approach to incident management. + Maintains confidentiality of information discovered through incident investigations, including but not limited to the confidentiality of records, and staff performance. + Completes other quality improvement related activities as assigned. Qualifications and Education Requirements: Master’s Degree in a Human Services or related field PLUS less than one year’s experience in human services. OR Bachelor’s Degree in a Human Services or related field PLUS one years’ experience in human services. OR Associate’s Degree in a Human Services related field PLUS two years’ experience in human services. OR High School Diploma or equivalent diploma PLUS three years’ experience in human services. PLUS A valid NYS driver’s license, approved driving record, and access to a vehicle is required. Preference will be given to individuals with investigation experience. Receive supervision instruction in a positive manner. Adhere to and administer Agency’s policies, procedures, rules and regulations. Maintain a professional attitude and appearance at all times. + Equipment Operation: The ability to operate computer, copier, fax machine and occasionally agency vehicle.
