Under the supervision of the Chief Quality Officer (System), the Quality Coordinator plays a vital role in our healthcare delivery system. Primary areas of accountability may include:
+ Creation and execution of hospital wide “Conversation Ready” Initiative
+ Point person for any ACP/EOL needs on patients without Palliative Care on board
+ Supportive role to all clinicians to ensure thorough ACP
+ Supportive role to all clinicians to provide the highest level of care to patients nearing EOL.
+ Ongoing training/involvement for designated “Conversation Champions”
+ Creation of educational and interactive resources for patients and their families to assist with EOL decision making
+ Identification of triggers to start palliative and hospice consultations earlier in the hospitalization
+ Ongoing clinician palliative concepts education
+ Conversation Ready educational classes
+ Skills Day Conversation Ready education
+ Development of Cerner documentation to capture ACP
+ Development of note templates to capture ACP conversations ? Continued process improvement/growth of the initiative
+ Data collection on impact of the initiative
+ Assist with goals of GIP hospice initiative
+ Nurse education
+ Nurse GIP conversion protocol
+ Hospice liaison
+ Supportive role to bedside nurses to provide highest level of care to patients under to hospice care
