Quality Improvement Coordinator I Job Description Analyze, develop, implement, and monitor clinical quality improvement initiatives to achieve healthy outcomes. Communicate with provider practices to ensure that transitions of care (TRC) and Medication Reconciliation Post-Discharge (MRP) have been completed and uploaded to the EMR. Audit medical records and monitor performance measures for healthcare risk management, sentinel events, and trends. Perform administrative tasks including requesting and processing medical records. Comply with quality improvement programs according to state requirements, support Quality Assurance Performance Improvement work plan/initiatives, schedule and assist with committee and sub-committee preparation, and assist in the investigation and resolution of member quality of care complaints. Responsibilities + Analyze, develop, implement, and monitor clinical quality improvement initiatives to achieve healthy outcomes. + Communicate with provider practices to ensure TRC and MRP have been completed and uploaded to the EMR. + Audit medical records to ensure transitions of care (TRC) and Medication Reconciliation Post-Discharge (MRP) are included. + Monitor performance measures for healthcare risk management, sentinel events, and trends. + Request and process medical records as part of administrative duties. + Perform duties to comply with state quality improvement programs requirements. + Support Quality Assurance Performance Improvement work plan and initiatives. + Schedule and assist with committee and sub-committee preparation. + Assist in the investigation and resolution of member quality of care complaints. Essential Skills + Quality improvement + Medical record auditing + Healthcare risk management + Electronic medical record (EMR) management + Transitions of care + Nursing clinical experience + Utilization management Additional Skills & Qualifications + Bachelor's degree in nursing preferred + 2+ years clinical, quality management or healthcare related experience + At least one year experience in a quality function within a healthcare setting + LPN, RN, PA, or LCSW license preferred + Utilization management certification preferred + Certified Professional in Healthcare Quality preferred Work Environment Hybrid work environment with 50% of the time reporting to the Columbus office and the remaining days working remotely. Travel to the Columbus office is required for 5 out of 10 days. A schedule will be developed post-onboarding to rotate and split in-office days, allowing for a flexible hybrid setting. Pay and Benefits The pay range for this position is $20.00 - $25.00 Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
