Job Summary
The Quality Improvement/ Family Team Meeting Facilitator will work with program staff to collect data and use evidence to develop measurable goals for program improvement using a Continuous Quality Improvement approach. This person will coordinate and facilitate Family Team Meeting while ensuring program compliance w/agency policies and government regulations from WDSS and NYS Office of Children and Family Services.

Key Responsibilities
+ Coordinate and Facilitate Family Team Meetings
+ Ensure WDSS child safety and risk elements are included in each meeting.
+ Develop action plans to best meet the needs of the family in a collaborative manner with their input.
+ Ensure proper documentation of meetings is recorded.
+ Work collaboratively with the program leadership to ensure preparation, coordination and follow up on external audit processes.
+ Conduct Case Record Review activities and prepare monthly reports.
+ Monitor and assist with implementing on-going QI activities including a systematic quality improvement plan which identifies program strengths and weakness.
+ Assist with the formulation, implementation and monitoring of corrective actions to ensure program improvement including but not limited to policy/procedure development and review, establishing benchmarks, training, etc.
+ Participate as part of the QI team in staff meetings, training, and team building activities.
+ Collect and assess program data and collaboratively work with the BSFT team around program improvement.
+ Assess reports and audits received from government jurisdiction and manage follow up.
+ Other duties as assigned.

+ You have at least a bachelor's degree with some OI/OA experience.
+ You are proficient in core competencies required: data collection, assessment, collaborative problem solving, facilitation, written communication, and strategic planning.
+ You have strong Microsoft Excel skills.
